Referrals

HHH offer a wide range of supported accommodation for adults 18-75.
Service users can be referred to us from local authorities and external agencies within Birmingham.
If you would like to apply for supported accommodation, please contact us and a member of the team will be happy to help. We are here to help those most in need.
When a referral is received the following assessment are taken into consideration:

•   Locality Report
•   Risk of harm towards others
•   Vulnerability
•   Emotional Coping Strategies
•   Educational Needs
•   Independent Living Skills to be developed
•   Assessment of young person / adults ability to integrate with current group
•   Summary Medical history

After an initial assessment, a decision is made regarding acceptanceof a referral. We do prefer where possible, that the young person or adult is involved in the referral process and is always invited to view the accommodation and meet face-to-face with staff and other service user within the home.

This allows us to get to know each other and further assess our ability to support them.
Service users are always encouraged to slowly take responsibility for their lives and build on their independence and living skills.

In cases where learning difficulties, behavioural issues, mental health and other issues are a factor we work with other professional agencies to help service users address and tackle any issues.
